By accessing our website, submitting an intake, or becoming a patient, you agree to these Terms. If you do not agree, do not use our services.
1. Acceptance of terms
By using our website or services, you agree to be bound by these Terms and our Privacy Policy. If you are using the services on behalf of another person or entity, you represent that you have the authority to bind that person or entity to these Terms.
2. Not an emergency service
Medical emergency notice
Forge does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use our website, messaging system, or contact forms to communicate a medical emergency.
Forge is a concierge performance medicine practice focused on proactive, personalized care. Response times to non-emergency communications vary by membership tier and are described in your membership agreement. Even on tiers that include expedited response, Forge is not a substitute for emergency care.
3. Description of services
Forge offers concierge performance medicine services under tiered membership programs (Forge Core, Forge Elite, and Forge Executive). Services may include, depending on your tier:
- Comprehensive diagnostic assessments, including blood panels, DEXA, VO₂ max testing, and related diagnostics.
- Physician-led consultations and ongoing care.
- Personalized treatment and optimization protocols.
- Wearable device integration and biometric review.
- Direct physician messaging or on-demand access, as specified by your tier.
Specific services, inclusions, and limits are described in your membership agreement. Services may be updated or modified at our discretion with appropriate notice to active members.
4. Eligibility
To become a patient, you must:
- Be at least 18 years of age.
- Be a resident of the United States.
- Be located in a state where our physicians are licensed to practice at the time services are rendered.
- Provide accurate and complete information during intake and throughout the course of care.
Forge physicians are medically licensed in all 50 states. Physical clinic services are available at our locations in Aurora, Colorado and Glenwood Springs, Colorado.
Acceptance as a patient is at the physician's discretion based on medical fit and scope of practice.
5. Physician-patient relationship
Submitting an intake form, scheduling a consultation, or browsing our website does not establish a physician-patient relationship. A formal physician-patient relationship is established only when you (a) complete the onboarding process, (b) execute a membership agreement, and (c) are accepted by a Forge physician.
Until a physician-patient relationship is established, communications with Forge are for informational purposes and do not constitute medical advice.
6. Membership and fees
Membership fees, billing cycles, and specific inclusions are described in your membership agreement. Unless otherwise specified:
- Membership fees are billed monthly or annually in advance, depending on the plan you select.
- You authorize us to charge your designated payment method on a recurring basis.
- Fees are exclusive of any third-party costs (lab fees, imaging, prescriptions, wearable devices) unless expressly included in your tier.
- Forge may adjust fees with at least 30 days' written notice. Continued membership after the effective date constitutes acceptance of the new fees.
Forge does not bill health insurance for membership services. You are responsible for understanding whether any of our services may be reimbursable through your insurance, HSA, or FSA.
7. Cancellation and refunds
You may cancel your membership at any time by providing written notice to [email protected]. Specific cancellation terms are defined in your membership agreement. Unless otherwise specified:
- Cancellation takes effect at the end of the current billing cycle.
- Fees already paid for the current billing cycle are not refundable.
- Annual memberships may have specific cancellation and proration rules described in your membership agreement.
Forge reserves the right to terminate any membership for non-payment, material breach of these Terms, or conduct that our physicians determine is incompatible with providing safe and effective care.
8. Telehealth services
Forge provides telehealth services to members via secure video and messaging platforms. By using telehealth, you acknowledge:
- Telehealth sessions are governed by the laws of the state where you are physically located at the time of the session.
- Technical failures (connection issues, platform outages) may occasionally interrupt sessions.
- Certain conditions require in-person examination and may not be appropriate for telehealth.
- Your physician will determine whether telehealth is clinically appropriate for your situation.
9. Your responsibilities
As a patient, you agree to:
- Provide complete, accurate, and current health information.
- Follow reasonable treatment recommendations from your physician or communicate when you cannot or do not wish to.
- Notify Forge promptly of material changes in your health status, medications, or contact information.
- Use secure messaging and other platforms for their intended purpose.
- Not share your account credentials or allow others to access your account.
- Comply with all applicable laws in using our services.
10. Intellectual property
All content on our website, including text, graphics, logos, images, and software, is the property of Forge or our licensors and is protected by copyright, trademark, and other intellectual property laws. You may not reproduce, distribute, or create derivative works from our content without our prior written consent.
Personalized treatment plans, protocols, and the "Forge Playbook" provided to you are for your individual use only and may not be shared, sold, or distributed.
11. Disclaimers
Our website and services are provided "as is" and "as available." While our physicians bring decades of clinical experience to every patient, medicine is not an exact science, and no specific medical outcome can be guaranteed.
Forge makes no warranties, express or implied, regarding:
- Specific health outcomes from our services or protocols.
- Uninterrupted or error-free operation of our website or platforms.
- The accuracy of third-party data, including wearable device data or lab results.
Information on our website is provided for general educational purposes and is not a substitute for medical advice tailored to your specific situation.
12. Limitation of liability
To the fullest extent permitted by law, Forge and its directors, employees, and agents shall not be liable for any indirect, incidental, consequential, special, or punitive damages arising from or related to your use of our website or services, even if we have been advised of the possibility of such damages.
Our total liability for any claim arising from or related to these Terms or your use of our services shall not exceed the total amount you have paid to Forge in the twelve (12) months preceding the event giving rise to the claim.
Nothing in these Terms limits liability for gross negligence, willful misconduct, or any liability that cannot be limited by applicable law. Claims related to medical care are governed by applicable medical malpractice laws and are not limited by this section except to the extent permitted by law.
13. Indemnification
You agree to indemnify and hold harmless Forge and its directors, employees, and agents from and against any claims, damages, liabilities, and expenses (including reasonable attorneys' fees) arising from (a) your breach of these Terms, (b) your violation of any law or third-party right, or (c) your misuse of our services.
14. Arbitration and disputes
Any dispute arising from or related to these Terms or your use of our services (excluding claims for medical malpractice or professional liability, which are governed by applicable law) shall be resolved through binding arbitration administered by the American Arbitration Association under its Commercial Arbitration Rules.
Arbitration will be conducted in Centennial, Colorado, unless the parties agree otherwise. The arbitrator's decision shall be final and binding. You waive the right to a jury trial and to participate in a class action.
Opt-out notice
You may opt out of this arbitration agreement within 30 days of first agreeing to these Terms by sending written notice to [email protected] with the subject line "Arbitration Opt-Out."
15. Governing law
These Terms are governed by the laws of the State of Colorado, without regard to its conflict-of-law provisions. Any claims not subject to arbitration shall be brought exclusively in the state or federal courts located in Arapahoe County, Colorado, and you consent to personal jurisdiction in those courts.
16. Changes to these terms
We may update these Terms from time to time. The "Last updated" date at the top reflects the most recent revision. Material changes will be communicated by email or a prominent notice on our website at least 30 days before they take effect. Continued use of our services after the effective date constitutes acceptance of the updated Terms.
